Head: sources near Sweeneysburg in Raleigh County.
Mouth: junction with Kanawha River at Pratt, WV in Kanawha County.

Paint Creek drains parts of the Cumberland Mountains region of western Fayette County, southern Kanawha County, and northern Raleigh County. The stream takes its name from trees that were painted by native Americans along part of a trail that shadowed the stream near Pax, WV. The West Virginia Turnpike carries Interstate expressways 64 and 77 along part of the stream. The Paint Creek Scenic Trail follows the stream for most of its length.
